Current students please complete the attached Transcript Request Release for each school you wish to receive your transcript.  If you are under the age of 18, a parent or guardian signature is required.

Please note that at the end of the school year seniors will be given a separate document requesting where to provide their final transcript.  A transcript release must already be on file for that school.

During the summer months, the completed form should be mailed to the high school or faxed to 608-296-2938. During the school year the form may also be given to your school counselor or the high school office. Please allow 3-5 days for processing.
