What the Office Needs for Student Athletes Participate in Athletics:
Please take a few minutes to review the Pioneer Plan Parent Code, the Concussion/Sudden Cardiac Arrest, WAHS/MS Code of Conduct, WIAA Eligibility using the links above.
Once you have familiarized yourself, and your athlete with the information, regulations, and expectations each link provides, please print and sign the "Athletic Signature Sheet."
The WAHS/MS office will need to have a copy of the "Emergency Medical Form" on file. Please print and sign this form as well.
Last but not least, The WAHS/MS office must have a current "WIAA Physical Form" on file in order for your athlete to participate. Physicals are good for two years. If your child has had a physical AFTER April 1st, 2023, then that physical is considered current for the 2024-25 school year. This form must be signed by a licensed physician. On alternate years, you'll need to fill out an "Alternate Year Card."
Please feel free to contact Mr. Daniels or Ms. Noe if you have any questions. You may also obtain copies of the full packet in the High School/Middle School office.
