What the Office Needs for Student Athletes to Participate in Athletics:

Please take a few minutes to review the Pioneer Plan Parent Code, the Concussion/Sudden Cardiac Arrest, and the Westfield MS/HS Code of Conduct using the links above.

Once you and your athlete have familiarized yourselves with the information, regulations, and expectations each link provides, please print and sign the "Athletic Signature Sheet."

The Westfield MS/HS office will need to have a copy of the "Emergency Medical Form" on file.  Please print and sign this form as well.

Last but not least, the Westfield MS/HS office must have a current "WIAA Physical Form" on file in order for your athlete to participate.  Physicals are good for two years.  In alternate years, you'll need to fill out an "Alternate Year Card."
